Advance Care Planning Documents Empower Your Medical Decision-Making

An advance directive is a legal document giving you a voice involving medical decisions in the event you are incapacitated. Many people shy away from creating such documents as they fail to realize how important they are, and mistakenly worry they are signing away their lives, and/or don’t like thinking about being so ill they are unable to make decisions.

Advance care planning documents may include a living will, power of attorney and health care proxy. By planning ahead, they guide decisions made on your behalf if you are unable, and suffer from an incurable, irreversible illness, condition or disease determined to be terminal. Living will specify the kind of medical treatment desired and many simply express the desire to not be given life-sustaining measures that only serve to prolong a person’s death. Specific details may regard care including the use of pain medications, antibiotics, hydration, feeding and the use of cardiopulmonary resuscitation or ventilators.

A health care proxy is someone designated to express your wishes for medical care and has the same rights you would in requesting or refusing treatment. A durable power of attorney document names a person to act on your behalf if you are incapacitated and gives them the power to make financial decisions.

The Harvard Medical School reports on some of the myths people have regarding advance care directives. They say you need to realize more care is not always better but can prolong the dying process without respect for quality of life or comfort. Refusing life support does not mean you are committing suicide but rather allowing your medical problem to be the cause of your death. Moreover, refusing life-extending treatment does not mean you are refusing comfort measures for pain management and symptoms. Talk to your family and doctor about your medical preferences, and be sure each has a copy of your advance directive documents.

Memory Care Communities Provide a Safe and Comfortable Environment

If a loved one suffers from Alzheimer’s or another dementia-related disease, at some point, you may require professional help. The number one concern at a memory care community is to ensure the safety of the person as many of the disease’s symptoms place the person more at risk for accidents and other mishaps.

Forgetfulness and confusion are some of the most recognizable signs of Alzheimer’s disease and can cause problems like forgetting to turn off a stove, getting lost in familiar areas or wandering. Other common issues in the early stages of the disease include vision/spatial issues and impaired reasoning and judgment, problem solving and difficulty in completing everyday tasks.

Memory care communities are equipped to provide a safe, stable residential environment for seniors with memory impairment as well as provide meaningful activities to keep the person engaged. At most communities, the staff receives extensive training and information regarding the disease to better treat and understand residents.

At MorningStar of Happy Valley, we offer a holistic approach to memory care for the utmost comfort, safety and well-being of the individual. Once a family decides to entrust the care of their loved one to MorningStar, we create a Personalized Service Plan to provide the care, people, programs, and activities they need. Residents receive healthy, specially prepared meals to make eating less stressful along with daily health monitoring, 24/7 care from a well-trained staff, access to licensed nurses and other third-party service providers.

Our Reflections Neighborhood has welcoming common areas and is designed to support residents’ independence as much as possible. To ensure the safety and well-being of residents, our community is completely secure, including an outdoor courtyard. In addition, all suites are equipped with emergency call systems and handicapped-accessible bathrooms; and we offer SafelyYou technology, which can detect and prevent falls for residents with (AI) Artificial Intelligence. Risky OTC Drugs for Seniors

All too often people think of over-the-counter drugs (OTC) as being safe and not having the side-effects or drug interactions their prescription drugs might have. Experts say this attitude could put you at risk as many OTC drugs are not as harmless as we think, especially for older adults. AARP recently cited a statistic showing as many as 178,000 hospitalizations a year are caused by a reaction to OCT medications and adults 65 years and older were more commonly harmed.

One reason older adults are more vulnerable to adverse outcomes are many already take several prescription drugs. A report from the Lown Institute, a non-profit organization, reported nearly half of adults 65 years and older take five (known as polypharmacy) or more prescription drugs daily. Another 20 percent over 65 take 10 or more daily.

This increases the risk of interactions when taking OTC medication for pain relief, stomach issues, allergies or other needs. In addition, the AARP article explains as we age, our bodies lose muscle and gain fat, which slows down the process of getting rid of medications. One of the organs responsible for processing medication is the kidneys, which start to work less effectively to filter and eliminate waste causing medications to build up. Aging livers also work less effectively to break down medications.

Common OTCs like acetaminophen (generic name for Tylenol) can cause damage to the liver. Ann M. Hester, M.D., a board-certified internal medicine physician based in Highland, Maryland explains overuse of acetaminophens is the leading cause of deadly liver failure. Seniors on blood thinners should be aware that ibuprofen (Motrin and Advil) and naproxen (Aleve) OTCs drugs may result in stomach bleeding and ulcers. Moreover, doctors no longer recommend people take a daily aspirin as a preventive measure toward heart disease as it too may cause stomach bleeding.

People with high blood pressure should be aware that decongestants containing phenylephrine or pseudoephedrine like Sudafed can increase blood pressure. It can also cause problems to combine an OTC pain reliever with a decongestant as it too raises blood pressure putting the person at risk for a stroke; and men with prostrate issues are at risk for urinary problems. There are other OTCs that seniors should be wary of so before you take one, be sure to check with your healthcare provider.

Hearing Loss and Cognitive Impairment

JAMA Neurology added to the growing body of evidence showing a link between hearing loss and cognitive impairment. The study found treating hearing loss with hearing aids may reduce the risk of developing dementia by up to 19%. In fact, researchers say hearing loss may be the biggest treatable factor for dementia. An article in AARP explains what may cause the link between hearing loss and dementia.

The first theory regards socialization. Scientists say once a person starts having more difficulty being in a group of people and following the conversation, they may avoid such social gatherings. This in turn can lead to isolation, which is problematic as less social engagement as well as less cognitively stimulating activities and depression are risk factors for dementia.

Secondly, coping with hearing loss means the person is constantly working to sort out sounds and make sense of what is being said. The brain now may have “fewer resources (brain power) to help preserve thinking and memory abilities.” Scientists also believe hearing loss causes the brain to be less stimulated with sound information, which is linked with parts of the brain shrinking and atrophying faster with chronic deprivation.

While experts agree we still need more studies regarding the link between hearing loss and a higher risk for dementia, they do know that addressing a hearing issue with hearing aids is beneficial for the person’s overall quality of life. Moreover, there are no negative medical issues associated with the devices.

Millions of Seniors are Treated for Falls Each Year

When an older adult trips and falls, it can be life changing. Consequences include serious head injuries, hip fractures and broken bones, and in the worst cases, fatalities. The Centers for Disease Control and Prevention (CDC) reports 3 million seniors are treated in emergency departments for fall injuries each year. Furthermore, more problems can arise as many seniors need several months to recover, which can cause a decline in strength and mobility as well as mental agility. It can also lead to the loss of independence and isolation.

Statistics show most falls happen for a handful of reasons and being forewarned may help prevent one. The environment, especially the person’s home and surrounding outdoor area is one of the biggest culprits. Danger spots include slippery bathtubs, high mattresses, steep stairs, poorly lit hallways, loose rugs, obstacles in walkways and clutter. Experts say a little bit of time spent addressing these problems with things like non-slip bath mats and grab bars, more lighting, removing clutter, etc. can help prevent a fall.

Another risk factor for a fall is chronic health conditions that affect balance and mobility. Arthritis, heart disease, diabetes and high blood pressure can mean a person might experience dizziness or be unable to control muscles to stop a fall.

Growing older also puts us at risk for diseases like glaucoma, age-related macular degeneration, diabetic retinopathy and cataracts, which in turn, leads to more accidents. Solutions include having yearly eye exams and making sure your eyeglass prescription is up to date.

Experts say it is important for seniors to stay as fit as possible in memory care as aging affects muscle strength, flexibility and coordination. Check with your doctor for information on balance and strength exercises. If a medication is making you dizzy or drowsy, talk to your doctor about it. There may be another drug just as effective that doesn’t have the same side effects.
